Moda Health offers a full range of pharmacy benefits, all designed to give patients maximum convenience and reduced out-of-pocket expenses. If you have any questions, please contact customer service.
Moda Health plans do not utilize a formulary, but many plans have a three tiered structure with lower co-payments for generics or preferred drugs.

Moda Health designs its formularies based on evaluations of efficacy, safety and cost-effectiveness of drugs. Our number one priority is always providing the member with the most effective treatment at the best price to help them get well sooner and live well longer.
For your convenience, we’ve provided the OHP and ODS Advantage formularies. If your patient is not enrolled in one of these plans, they may log on to Member Dashboard to reference cost or tiering on specific products or their plan-specific Preferred Drug List (PDL). Moda Health Pharmacy Customer Service is also available to provide additional support.
